During your first visit, our objective is to get to know you better, to become familiar with your past dental experiences and to establish a treatment plan that will best meet your dental needs.
When you arrive, please be prepared to complete all insurance and health information forms if you haven’t already done so by downloading the online forms and completing them ahead of time (please see download links below for New Patient forms). We ask that you fill out several forms that will get you acquainted with our office. Also, please assist us by providing the following information:
• Any panoramic or full mouth X-rays taken within the past 3 years.
• If you have dental insurance, please bring your insurance card. This will enable us to give you an estimate of your costs.
Your initial appointment will last approximately one hour. In most cases, we will clean your teeth and X-rays will be taken. Our dentist will perform a thorough exam and evaluate the diagnostic records, which help in diagnosing teeth decay, periodontal problems, jaw problems and any signs of oral cancer. After the examination, our dentist will provide a detailed summary that outlines any existing dental problems and proposed treatment. Take-home oral hygiene instructions will also be provided, along with any suggestions to help you care for your teeth.

It can really be a irritating to sit in a waiting room longer than needed filling out forms before your appointment. We’d like to prevent this inconvenience by having our forms online so you can complete them at your convenience, before you ever get to our office. Just follow the links below to download the appropriate form (adults are 18 years old and older), print them out and write in all the information. All that’s left to do is hand them to our office manager and you’re ready to go.

Your scheduled appointment time has been reserved specifically for you. We request a 48-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.
It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard, American Express and Discover credit cards.
We also offer low- and no-interest financing options through the CareCredit system*. If you have any questions about financing or payment, ask us! We will thoroughly explain your choices and work hard to accommodate your needs.

If your insurance covers dental treatment, you will receive the benefit of reduced personal costs. For orthodontic treatment like Invisalign®, please keep in mind that many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Insurance policies vary, so we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. Please remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.
